Sage of the Six Paths mod is essentially an upgraded version of the original Sage form, not only in terms of power, but also in design. As Naruto puts on the chakra robe thanks to Kurama, he now has a large shiny cape, which contrasts with the black undershirt that has faint markings of the form's symbols on his first thing.

There is obvious attention to detail in this form, such as Naruto's whiskers turning into giant black check marks, and clothes that fit him better as he ages.
