Unlock the back gate and find your way inside
Go through the gate and defeat the enemies in the sewer area.
How to open the 3-digit code door in the Curdsville Nuthouse
A door in the northeast corner of the second sewer room leads to a door that is unlocked with a three-digit code. The correct three digit code is 4-5-1. Use the levers on the wall to set the code and press submit to open the door. For more information on this puzzle, see our One Flue Door Code Guide.
Opening this door does not need to be 100% complete.
Now return to the second sewer room and climb the ramp to the west. Go through the vent at the top.
Look for anything suspicious
Alternative clues
Enter the prison area and defeat all the enemies you find there. Climb the ladder and use the wall jump to reach the open door to the office. There is an alternate clue on a desk in the office.
Shoot the barrel on the floor in the office to destroy the floor. Fall through the hole and flip the switch when you land. Follow the path through the prison to enter the decontamination chamber.
Talk to Millie and follow Millie
When the door to the decontamination chamber opens, approach Mili and talk to her. Shoot the target above his head and enter the water filled area.
Planned
Jump into the water and open under the bridges that allow you to swim to the central structure. Follow the path inside, and you'll find a schematic sitting on top of a crate.
Exit the central structure and return to the bridge at the beginning of the area. Use wall jumps on the walls on the platforms held by the statues to cross the area, rotate them by shooting the targets on the walls. At the last bridge, shoot the target and enter the warp-pipe.
Now flip the switch in front of you and cross the bridge. Flip the switch to your left and continue forward for Millie. Go ahead.
Talking to the brain? Go to Inkwell
Planned
Talk to Jar-Head and take the schematic from the chalkboard in the northwest corner of the room. Then go down the path, flip the switch, and drop into the area where you kick the skeleton onto the surgical table. Get to the brain.
How to fill the Inkwell
When the fight starts, look in the corners for a surgical table with an active pen. Get the enemy close to that corner and kick them into the table. Repeat this process until the encounter is complete.
Transfer the ink to the brain chamber, return to your little overlord, and collect the sentient gun
Go back to the switch you flipped before going to the inkwell area and flip it again. Go back to the room where you first met the brain and talk to it. Take the jar-head gun from the opening case.
Go FFF-Forth, talk to Millie again?, and put Millie back
Comic book
Go forward and cross the warp-pipe by the typewriter. Look through the opening gate to your right to find a comic book in a wheelchair.
Planned
Now go forward and climb the stairs. In the next area, look for the schematic on top of the crate on the right side. Grab it and return to Millie.
Talk to Millie and shoot the target in her head. Cross the bridge and look left to find a piano.
How to solve the piano puzzle in the Curdsville Nuthouse
Go to the piano and press CAGE on its keys. Cross the bridge below and follow the path to find Gloria in a cell. Take her photo. For more information on the piano puzzle, check out our Nuts side mission walkthrough.
Continue following Millie, and shoot the targets on the moving birdhouses as they appear. Cross the bridge below and go to the gate with the button. Press the button and go ahead.
Defeat the enemies in the next room and flip the switch below to open the gate. Jump to the trampoline next to the gate and jump to the next switch. Wall jump to cross the gap and go through the laundry area, flipping another switch and shooting the target to open the giant washing machines.
Talk to Millie once and find the elevator
Baseball card
Exit above the laundry room and enter the warp-pipe by another typewriter. Go through the gate that opens on your left and jump onto the pipe on the left wall. Cross the platforms to reach a warp-pipe that leads to another area. Follow the path to the gate at the end and look in the cabinet on your right to find a baseball card.
Planned
Now go back through the Warp-Pipe and talk to Millie. Shoot the target above the gate to your right and continue forward. At the wooden section of the path, jump onto the banister and jump to the top of the central structure. Here you will find a planned safe.
Return to the path and flip the switch to lower the bridge used to enter the central structure.
Climb and boldly go where no mouse has gone before
Go inside the structure and enter the elevator in the middle. Flip the switch to catch the ride and go into the portal. Cross it to complete the One Flew mission.
