Players must progress through the new game in New Game Plus if they want to find the items needed for the UFO ending.
Pick up the blue gem at the east end of South Vale's Jewelers
Once players have defeated their first enemy and are free to roam the east end of South Valley, head up Blue Street and go behind Big J's. A building to the right will contain a jeweler, and smashing the display window to the right of the shop will reveal an open wooden box containing a blue gem. Just interact with it to pick it up.
Use the blue gem on the Saul Apartments Rooftop
With the blue gem in hand, players must head to Saul Street, still on the eastern side of South Vale, and venture through the Saul Street Apartments until they reach the rooftop. Once on the roof, players will briefly hear a strange sound. Players can enter the list and select their main items, where they want to check the blue gem. Doing so at this location causes the Blue Gem to emit a sound and bright light, indicating that players have correctly activated the first location to use the Blue Gem. Now, progress past the Blue Creek apartment until you meet Maria.
Use the blue gem on the pier in Rosewater Park
Once players meet up with Maria behind the Blue Creek Apartments, they'll want to immediately head to the right area of Rosewater Park. While standing on the Rosewater Park Pier, enter the main items on the list and click Check on the Blue Gem. This should emit another bright light and a strange sound from the gem. Players can now progress through the West Side and most of South Valley Silent Hill 2Chapters until reaching the Lakeview Hotel area.
Use the blue gem on the dock at the Lakeview Hotel
Once players cross Toluca Lake and enter the Lakeview Hotel dock, players will want to immediately enter their inventory, take out the blue gem, and check it out like twice before. Doing so emits a familiar sound and beam of light, and now players can progress through the Lakeview Hotel chapter until they solve the music box puzzle.
Use the Blue Gem in Room 312 of the Lakeview Hotel
After completing the music box puzzle, players can take the key to 3F and enter room 312. However, players do not want to use video tape. Instead, once players enter room 312, pull out the blue gem a fourth and final time and examine it. After checking it out, players will immediately enter a cutscene that will show them the UFO ending, and the credits will roll soon after.

The UFO End, Explained
When James ignores the videotape of room 312 and instead goes outside to the balcony, he manages to attract the interest of a flying saucer above. It then adapts the ending to a pre-1940s TV serial, with the story told through black-and-white images and text, where Silent Hill 2 (2024) James Sunderland meets… Silent Hill 2 (2001) James Sunderland?
The PS2/original James is called Space James and informs the PS5/remake James that he is a clone. A classic little gray alien taps James, and both the alien and Space Gems take James to their spaceships, back home, wherever. PS2 James gazes longingly at the Lakeview Hotel, uttering one last word before the credits roll: “my…“
The ending is as silly as it gets, with a meeting of James' original PS2 render and a modern PS5 model. This is a joke ending that shouldn't be taken seriously, but by all means, if players want this to be their canon ending. Silent Hill 2 playthrough, then it is.
